Subject: Important Notice – Practice Closure
Dear Valued Patient,
We are writing to inform you that Evolution Healthcare, PLLC will be no longer seeing patients as of April 30, 2026.
What this means for you:
- You will need to establish care with a new healthcare provider to ensure continuity of care.
- If you require prescription refills, please submit your requests as soon as possible, as we will only be able to process refill requests until 4/30/26.
- Your medical records will remain available through the patient portal for your access during your transition to new provider. Please contact us at 630-549-5348 if you require assistance.
Current patients with active prescriptions have been contacted by phone with this information, and notification has been sent to all of our patients as well.
